DESCRIPTION:Please join us for our annual Fall Moms Night! This is a great opportunity to get to know other Pack 220 families before our fall campout (November 1-2)! Come enjoy a bite to eat\, connect with friends and have a fun night out!\n\n\nNOTES:\n\n\n- NO Scouts\, Uniforms or Pack T-Shirts at this event!  \n\n\n- Light appetizers will be provided but you are welcome to purchase additional food and drinks on your own\n\n\n- Please pass on this notice if Mom or other Adult Partner is not listed in Scoutbook. If you would like to add another adult to your Scout's account so they also receive messages\, please view our SCOUTBOOK FAQS on our website (https://bit.ly/lafayettecubscouts)\n\nYOU MAY BRING TO THIS EVENT: \n\n1) MEDICAL FORMS & INSURANCE CARD COPIES (REQUIRED to attend Fall Campout 11/1-11/2)\n\n\n- Medical forms and insurance card copies must be submitted for EACH FAMILY MEMBER (Adults\, Scout\, Siblings)\n\n\n- Forms are available at the bottom of the FAQ page on the Pack website: https://bit.ly/lafayettecubscouts\n\n\n- This form is collected annually\, so it is recommended you complete and save the fillable PDF the first time and SAVE A COPY so you can update as needed. Pack 220 will not be storing the forms digitally.\n\n\n- For Individual Insurance Cards (i.e. Kaiser)\, please attach a copy of the card to the respective form. If you have 1 card for the whole family\, please attach the same copy to each form.\n\n\n- Returning members must also update and resubmit medical forms.
